You could always just pass through a usb soundcard. This is what I do on my
main working Windows machine. I have a box that runs Debian on the bare
metal, and I run my Windows VM inside that via Virtualbox. However I was
not happy with the latency of the audio from my screen reader, so I picked
up a $5 usb soundcard dongle, and I pass that usb device directly through
to Windows. It works great, and I imagine that doing something similar
would almost certainly work for you, regardless of the guest OS. As long as
it can talk directly to a USB sound card, you should be good.

Good luck.
