When focused on the message in the message itself, open the file menu.  You'll find an attachment sub menu from which you can open or save the attachments.

Sure hope someone out there is using outlook 2003. I've just started it and
am having a lot of grief.
 
Can you tell me how to open an attachment or zip file in incoming mail? 
 
One problem is that while the e-mail says I have an attachment or zip file,
no further information is available in the e-mail. I'm using window eyes 6.1
but it doesn't tell me what the attachment link is.
