Another thing, which might work is, hold the camera in your left hand, if 
you're right handed.  I balance the camera with my pinky, ring and middle 
finger under the PDA, below the camera.  Then I point my index finger along 
the side of the camera/PDA, to help me hold it more level.  My thumb is on 
the top of the PDA for balance.  Then, after I have the desired height, I 
press the button to shoot the picture with my right index finger.

Pam. this may not work for you but it sometimes helps me. Put the camera
flat on the page, and when you are ready to shoot, bring the camera straight
up to the height you think you need and shoot. Try placing your documents on
a surface where your distance is predictable - maybe your lap - that works
for me. Then figure out what part of your body makes that 18 inches. For me
though the key is to put the camera level on the paper so I'm not guessing
at level, bringing the camera up, and then shooting before I have time to
get nervous and jiggle the camera.

Others may have better advice.
