Steve,

You print the result and return it in your regular mail-in ballot's
envelope.  The HTML can simply be sent in place of the marked or punched
cards as your particular county requires.  Someone then transcribes
answers to the form that can be automatically counted by the county's
machines, and the resulting traditional ballots are counted in the
traditional manner.
