[Journalists] questions concerning interviews

I'm looking into doing telephone interviews over the summer, and was wondering if anyone on this list thinks there are certain questions that should be asked before the interview - what sorts of descriptive questions should I ask?
  Also, ... since I'm requesting interviews via snail mail, what should the letter of request look like?  I'm planning on sending a letter requesting an interview as well as a list of the questions I'd like to ask.  is there anything else I should include?  
  Do I only address myself as being a student majoring in Broadast (since I have no official job ties to particular networks or news affiliations)?
