3. POSITION SUMMARY:
Coordinate Community Development Block Grant (CDBG) Home
Modification Programs and
perform assigned responsibilities in the Community Integration Unit.
Work closely with City
CDBG contract monitors, consumers and contractors to achieve home
accessibility. Monitor
contract compliance. Works directly with people with disabilities to
support their efforts
to adapt or maintain an independent lifestyle. Complete assigned
responsibilities with
emphasis on assessing consumer needs, developing goals, and providing
independent
living skills instruction. Persons with disabilities encouraged to

4. MINIMUM REQUIREMENTS:
Combination of 5 years of education and experience. Bachelor's
degrees must be in social
sciences, architecture or related fields. Experience must be in
provision of direct Independent
Living services, preferably with people with disabilities, project
coordinator and/or
architectural construction.
5. MAJOR ACCOUNTABILITIES:
A. Responds to initial contacts by consumers, contract service
providers, family members, social service agencies and community at
large.
B. Assesses and documents consumer Independent Living goals and home
accessibility needs, progress and activity in consumer files, with
emphasis on Independent Living skills training.
C. Identifies additional goals of consumer and makes referrals to other
ABIL programs or services in the community as appropriate.
D. Monitors home modification contracts, solicits bids for recommended
modifications and authorizes construction.
E. Monitors and evaluates contract budget. Ensures proper allocation
of funding components.
F. Monitors CDBG outcomes and submits program summary reports and
billing information as required.
G. Assists Executive Director and Unit Director to acquire additional
matching funding and in-kind construction contributions for home
modifications.
H. Develops, implements and updates procedures to facilitate contract
compliance and program goals.
I. Documents consumer contacts, consumer progress and program
activity, and submits billing information and monthly activity reports
as required.
J. Provides skills training, peer counseling/support, social and
recreational assistance, and outreach as needed.
K. Performs other duties as assigned.

Bachelor's degree in social sciences or related fields, or combination
of 3 years of education and experience. Experience must be in the
provision of direct services, preferably with people with disabilities.
5. MAJOR ACCOUNTABILITIES:
A. Respond to initial contacts by consumers, contract service
providers, family members, social service agencies, and community at
large.
B. Track incoming I&R calls and activities (follow-ups etc.)
C. Make referrals to ABIL programs and/ or community services as
appropriate and provide follow-up to meet individuals' I&R needs and
ensure that they are served.
D. Maintain and update ABIL databases and other agency resource
lists.
E. Assist ABIL staff with I&R questions as they pertain to their
programs.
F. Conduct outreach to community re: ABIL programs through
presentations, community meetings, health fairs etc.
G. Maintain accurate documentation of activities utilizing computer
database.
H. Complete and submit required billing information and monthly
activity reports as required in a timely and thorough manner.
I. Perform other duties as required.
