Dear Talking Book and Braille Service Patron:

 

Please consider taking a few minutes to complete the survey.  Your feedback
is important to us and helps TBBS identify areas for improvement.

 

 

There are several options for completing the survey.  Please choose the
option that works best for you.  

 

Option 1 - Complete the online TBBS Survey

Option 2 - Complete the survey by telephone with a TBBS staff member.  To
complete by phone just call (217) 785-0022 or 
(800) 426-0709 during our business hours, Monday-Friday 
8:00 am - 4:30 pm.

 

Option 3 - Request a large print copy of the survey and we will mail it to
you with a return envelope included.  To request a paper survey call TBBS at
(217) 785-0022 or (800) 426-0709 or e-mail isltbbs at islsos.gov
