Another possible bug is that when I am typing, voiceOver will never say the E when I put my finger on it, but will only say the phonetic "echo".

Hello.
I believe there's a bug in the latest version of the iPhone iOS. When selecting a ringtone, a page command (3-finger flick up or down) takes one clear out of Settings. Temporary fix is to read past a page boundary with VoiceOver.
