[gui-talk] form scanning software

I am looking for a  method of scanning in a pre printed form and being able to fill it out then print the completed form in its original format.  I see the Omni form product.  Is anyone using the product with screen reading software and how well does it work or is there other form scanning software out there that works well with screen readers?
