Another thing that I have found helpful with large texts is to have someone 
use Adobe Acrobat Creator, or another program, to break the textbook down 
into a separate file for each chapter.

Also, some publisher's texts are in protected PDF format.  I believe that 
when you open the file in Acrobat Reader it will say something like "Secure" 
or "protected" in the title bar of the application.  This can sometimes 
cause problems for Assistive Technology.
