Recently having need of a verifiable signature I retained the services of a
California notary public who established my identity on the basis of
personal knowledge and noted my passport and DMV identification card before
taking my signature.  The notary also indicated in her log that I was
visually impaired. 

In the state of Oregon, there are rules by which a notary can take the
signature of a blind person using a signature stamp Notaries Public Chapter
194, see 194.578 Use of signature stamp by blind or disabled person; rules 

Below there is a form from the State of Florida


 (Print, Type, or Stamp Commissioned Name of Notary Public) 

Personally Known __________ OR Produced Identification __________

Type of Identification Produced

(14)  A notary public must make reasonable accommodations to provide
notarial services to persons with disabilities.

(a)  A notary public may notarize the signature of a person who is blind
after the notary public has read the entire instrument to that person.

(b)  A notary public may notarize the signature of a person who signs with a
mark if:

1.  The document signing is witnessed by two disinterested persons;

2.  The notary prints the person's first name at the beginning of the
designated signature line and the person's last name at the end of the
designated signature line; and

3.  The notary prints the words "his (or her) mark" below the person's
signature mark.

(c)  The following notarial certificates are sufficient for the purpose of
notarizing for a person who signs with a mark:

1.  For an oath or affirmation:

 (First Name)  (Last Name) 

 His (or Her) Mark 


STATE OF FLORIDA
COUNTY OF __________


Sworn to and subscribed before me this _____ day of __________,  (year) , by
(name of person making statement) , who signed with a mark in the presence
of these witnesses:

Hi everyone,

Has there been any legal issue with a blind person verifying their 
signature?  If there were a dispute and it were necessary to confirm a 
signature or deny the signature, has there been a way that a blind person 
would do that?
