Dear Arizona Association of Blind Students List Members,

You have been invited to take part in a study to learn more about factors
that contribute to the accessibility of on-line course content and of
university web sites to students with visual impairments or dyslexia. This
study will be conducted by Annmarie Gemma, School of Continuing and
Professional Studies, Department of Management & Systems, New York
University, as part of her master’s thesis work. Her faculty sponsor is Dr.
Louis Stern, NYU School of Continuing and Professional Studies.

If you agree to be in this study, you will be asked to take part in an
on-line survey about your experience in accessing on-line content at your
university while using assistive technology. You must be at least 18 years
old to participate. In addition, you must have used a learning management
system, such as Blackboard or WebCT, in at least one course, and have used
assistive technology while accessing the system. Participation in this study
will involve approximately 20 minutes.

There are no known risks associated with your participation in this research
beyond those of everyday life. Although you will receive no direct benefits,
this research may help the investigator understand web accessibility better
and may help the investigator to make suggestions to universities for
improving web accessibility for students who have visual impairments or
dyslexia.

Participation in this study is voluntary. You may refuse to participate or
withdraw at any time without penalty. You have the right to skip or not
answer any questions you prefer not to answer. Nonparticipation or
withdrawal will not affect your grades or academic standing.

Your individual responses will remain confidential. Your name or specific,
personally identifying features will never be associated with your responses
in any written or oral presentation. Instead, your responses will serve as
data that are grouped with the responses of other students.

If there is anything about the study or your participation that is unclear
or that you do not understand, or if you have questions or wish to report a
research-related problem, you may contact Annmarie Gemma at (718) 248-9689
or by e-mail at alg230 at nyu.edu or the faculty sponsor, Dr. Louis Stern at
(646) 312-3653 or ls105 at nyu.edu. For questions about your rights as a
research participant, you may contact the University Committee on Activities
Involving Human Subjects, New York University, (212) 998-4808 or
humansubjects at nyu.edu.

Please print a copy of this form for your records.

If you wish to participate, please select the following link and kindly
complete the survey by August 17th:

Your time and participation is greatly appreciated.

Sincerely,
Annmarie Gemma
